Referring Providers

To refer a child for Applied Behavior Analysis (ABA) services, please download and complete the ABA Referral Form. Once completed, fax the form to 530-362-5400.

Referrals may be submitted by medical providers, allied health professionals, or other service providers. Parent/guardian authorization is recommended to allow follow-up and coordination of care.
